AWARD DESCRIPTION

ADHA, in conjunction with Colgate, are pleased to present the Student Community Service Award which recognizes a student member chapter that has demonstrated outstanding community service activities.

GUIDELINES

Award recipient will be notified in April 2018 after judges review has been completed. Presentation to the award recipient will be made at ADHA’s at the 95th Annual Conference in Columbus, OH, during the Saturday General Session on June 23, 2018.

DESCRIPTION

This award is presented to a student member chapter that has demonstrated outstanding community service activities.

APPLICATION CHECKLIST

  • Complete application cover page
  • Complete seven sections of application to describe your project
  • Complete the roster listing all students in your class
  • Save a copy of the application to your computer
  • Print a copy of application for your records
  • Submit application by March 2, 2018

Please note, this application must be completed by a current student member. The project must have been implemented by an ADHA Student Chapter. Additionally, ninety percent (90%) or more of the students in your class must be student members of ADHA. For the roster, please provide student membership ID numbers. Students may locate their ID number on their membership cards or by contacting ADHA Member Services at (312) 440-8900. 

JUDGING

The Committee on Student Relations selects one ADHA Student Chapter winner per year for this prestigious award. Committee decisions will be final.

AWARD

One student chapter will be selected. The winning chapter will receive the following:

  • $3,000 cash award for the student chapter
  • An engraved award
  • Recognition in Access Magazine, ADHA's official publication
  • Recognition at ADHA's 95th Annual Conference in Columbus, OH, during Saturday's General Session on June 23, 2018
